Whilst the greatest effort has been made to ensure the quality of this text, due to the historical nature of this content, in some rare cases there may be minor issues with legibility. I found myself very impatient with this questionnaire but until the invitation to write this paper came, I did not really stop to analyze what my feelings were all about. What I then realized is that the source of my impatience and irritation was the very format of the questionnaire because it implied things about OD that I have never accepted and that reflect, in my view, a real corruption of what was originally a grand vision.
